Rehabilitation


The rehabilitation service provides specialist care and support for adults who have lost physical capabilities through illness or injury.

Rehabilitation services are offered in patients’ homes and there is also a dedicated inpatient unit at St Pancras Hospital. The emphasis is on working with patients and carers, setting and attaining goals that will enable each individual to regain skills and some element of independent living.

Community rehabilitation services

The Camden REACH team provides care and support to help adults who have complex or chronic diseases, stroke, brain injury, a fall  and who need extra help to live independently. 

The team works with patients and their relatives or carers. The aim is to promote health and independence in mobility, communication, work, leisure and everyday activities. The team often helps patients through the transition as they return home from hospital. They can also provide support to help avoid people being admitted to hospital for practical rather than medical reasons. The team is made up of many healthcare professionals including physiotherapists, occupational therapists, speech and language therapists, doctors and nurses.

Residential home rehabilitation

The rehabilitation unit at Ingestre Road provides up to six weeks’ residential care for Camden residents who are medically able to leave hospital, but unable to return home immediately.

Inpatient rehabilitation unit

The Inpatient rehabilitation unit at South Wing, St Pancras Hospital provides treatment and support for patients whose physical abilities have been decreased through illness – for instance, by stroke, a fall, or a musculo-skeletal condition. The care team support patients to regain everyday life skills, to return to independent living and to manage their disability with a positive but realistic outlook.

Patients are usually referred to rehabilitation from local acute hospitals.
